Factors Associated with Non-Adherence to Antiretroviral Therapy in Luanda During the Test and Treat Strategy: A Cohort Study

Resumo

Introdução: A adesão à terapêutica antirretroviral (TARV) é crucial para o controlo da epidemia de VIH/SIDA. Este estudo avaliou os factores associados à não adesão à TARV em portadores do VIH aos 12 meses de início de tratamento durante a estratégia Testar e Tratar em Luanda, Angola.

Métodos: Estudo de coorte retrospectivo que analisou dados de pacientes que iniciaram TARV em fevereiro de 2018 em duas unidades de saúde de Luanda. A adesão foi avaliada aos 12 meses. Foram analisados dados sociodemográficos e clínicos, e utilizou-se regressão logística para identificar fatores associados à não adesão.

Resultados: A coorte incluiu 81 participantes. A não adesão aos 12 meses foi de 45,7%. A idade mais jovem (18-28 anos) (OR=3,13; p=0,03), o seguimento no Hospital de Saúde de Luanda (OR=3,06; p=0,02) e a hospitalização prévia (OR=3,78; p=0,02) foram associados a maior probabilidade de não adesão. O seguimento no Hospital Geral dos Cajueiros mostrou-se associado a menor não adesão (OR=0,32; p=0,02). A taxa de adesão diminuiu ao longo do tempo, e a realização da carga viral associou-se à maior adesão.

Conclusão: A não adesão à TARV continua a ser um desafio significativo. Os resultados destacam a importância de intervenções direcionadas a pacientes mais jovens e em locais específicos de seguimento, bem como a pacientes com histórico de hospitalização, para melhorar a adesão e o sucesso da estratégia Testar e Tratar em Angola.

Abstract

Introduction: Adherence to antiretroviral therapy (ART) is crucial for controlling the HIV/AIDS epidemic. This study evaluated the factors associated with non-adherence to ART in people living with HIV at 12 months after initiating treatment during the Test and Treat strategy in Luanda, Angola.

Methods: A retrospective cohort study analysed data from patients who started ART in February 2018 at two health facilities in Luanda. Adherence was assessed at 12 months. Sociodemographic and clinical data were analysed, and logistic regression was used to identify factors associated with non-adherence.

Results: The cohort included 81 participants. Non-adherence at 12 months was 45.7%. Younger age (18-28 years) (OR=3.13; p=0.03), follow-up at Hospital de Saúde de Luanda (HSL) (OR=3.06; p=0.02), and prior hospitalisation (OR=3.78; p=0.02) were associated with a higher likelihood of non-adherence. Follow-up at Hospital Geral dos Cajueiros appeared to be associated with lower non-adherence (OR=0.32; p=0.02). Adherence rates decreased over time, and having a viral load test was associated with higher adherence.

Conclusion: Non-adherence to ART remains a significant challenge. The results highlight the importance of targeted interventions for younger patients and those attending specific follow-up locations, as well as patients with a history of hospitalisation, to improve adherence and the success of the Test and Treat strategy in Angola.
